Free Printable Headache Diary Template

One row for each day of the month, plus a summary page to take to your doctor. Free, no sign-up.

Print in landscape. Each copy covers one month: two pages of daily rows and one summary page. The template is in English.

First page of the printable headache diary: one row for each day, with columns for headache, start time, duration, severity, symptoms, medicine, relief, sleep and notes

What you record each day

  • Headache or no headache. Fill in the days without one too; they show the pattern.
  • When it started and how long it lasted.
  • Severity from 0 (none) to 10 (the worst).
  • Symptoms: nausea, sensitivity to light or sound, and aura.
  • The medicine you took, the dose and whether it helped.
  • Your sleep, and what happened in the hours before: meals, caffeine, alcohol, stress, screens, weather or your period.

How to use it

  • Fill in a row every evening. A day without a headache takes ten seconds.
  • Write down the start time and severity during the headache if you can. They are the easiest details to forget.
  • Keep going for at least two months before you decide what your triggers are.
  • At the end of each month, fill in the summary page and take it to your doctor.

Watch your medicine days

Taking headache medicine too often can cause more headaches. Talk to your doctor if you take a triptan or a combination painkiller on 10 or more days a month, or a simple painkiller on 15 or more days a month.
